I think that over all, it's a good fit. I agree about the cuffs. You can try pinning to get them spot on for size and have the maker subtract that amount from the next order. I have had the opposite problem and now wear the largest wrist watch I'll wear when fitted (although it's been a while. The back is too large. There are different ways to fix that. one, obviously, is to have them use less material in the next order. Another is to dart the back (which is an option with the shirt you're wearing. I've done that w/ OTR shirts back before I learned about MTM shirts. The darts were a trademark of the now defunct "Custom Shop" shirts. WW Chan simply used less material.

I'm fine with it save wanting a slightly tighter cuff. It's comfortable.

I wouldn't mind trying to take in the chest and midsection some though and see how it feels.

Worth a try, but give it a few wearings before making any conclusions. A slimmer cut will feel "tight" at first, even if properly fit.
